Hag’s Eye

Wondrous Item • Common • Requires Attunement

To attune to the eye, you must gouge out your own eye and press it into the empty socket. The eye grafts itself to your head. While it is embedded in your eye socket, you can see through it as though it were your natural eye. You can insert or remove the eye as an action, and it can't be removed against your will while you are alive.

Hag’s Sight. You gain darkvision within a range of 15 feet. If you already have darkvision, the eye increases your darkvision's range by 15 feet instead.

Curse. This eye is cursed. Any creature within 1 mile of it can use it as a scrying focus; thus, a scrying spell targeting you automatically succeeds.

Hag’s Stare. You can cast witch bolt as a bonus action using this eye. When you do so, you can also use a bonus action to trigger the spell on your subsequent turns. Once used, this property can’t be used again until the next dawn.

Hag’s Stare. You can cast witch bolt (level 2) as a bonus action using this eye. When you do so, you can also use a bonus action to trigger the spell on your subsequent turns. Once used, this property can’t be used again until the next dawn.

Hag’s Vision. Once per day, you can cast the clairvoyance spell (sight only) as action using this eye. When you do so, you have truesight with a radius of 30 feet centered on the spell's sensor.

Hag’s Stare. You can cast witch bolt (level 4) as a bonus action using this eye. When you do so, you can also use a bonus action to trigger the spell on your subsequent turns. Once a spell is cast with this weapon, you can’t cast that spell from the eye again until the next dawn.

Hag’s Vision. As an action, you can remove the eye, set it on the ground. You can still see through the eye as normal if you wish. Additionally, you can use a bonus action to recall the eye while it is within 300 feet of you. The eye then teleports into your eye socket.

While it is embedded in your eye socket, you can see through it as though it were your natural eye. You can insert or remove the eye as an action, and it can't be removed against your will while you are alive.

Hag’s Glare. You gain advantage on checks using the Intimidation skill.

Hag’s Sight. You gain truesight within a range of 120 feet.

Hag’s Stare. Once per day, you can cast witch bolt (level 6) and eyebite as a bonus action using this eye. When you do so, you can also use a bonus action to trigger the spell on your subsequent turns. Once a spell is cast with this eye, you can’t cast that spell from the weapon again until the next dawn.

Hag’s Vision. As an action, you can remove the eye, set it on the ground. The eye has a movement speed of 5 feet and a climbing speed of 5 feet. The eye can climb difficult surfaces, including along ceilings, without needing to make an ability check. You can still see through the eye as normal if you wish. Additionally, you can use a bonus action to recall the eye while it is within 1000 feet of you. The eye then teleports into your eye socket.

Make a Deal. By spending 10 minutes, you can try to establish a telepathic link to a powerful grandmother hag. Roll a d100; if the result is lower than your level, the hag shows interest. Whatever the hag is offering you (boon, trinket, etc.) instantly appears on your person the moment the deal is struck. Similarly, everything you offer in return for it (hair, nails, good luck, etc.) disappears from your person. The GM has great leverage as to what the hag offers and demands. If you manage to establish a link with a hag, regardless of whether the deal is successful or not, you can use that trait again for 10 days.

Using Evolving Items

Evolving items can be used as standard magic items—the GM simply determines the rarity at which the item is found...

  • Character Growth. Reaching a specific level or proficiency milestone.
  • Arcane Empowerment. Enhancing the item through crafting or rituals.
  • Attunement Over Time. Deepening attunement through continued use.
  • Thematic Achievements. Fulfilling tasks or trials related to the item’s nature.
  • Divine or Infernal Favor. Gaining recognition from powerful entities.
  • Environmental Influence. Exposure to special locations or events.

By using evolving items, the GM can weave them into the story, making them feel personal and rewarding as they develop alongside the characters.
